Drop-off Requirements

Before You Submit Samples

1. Manifest

This document must include...

  • Client Information license number, business name, address, phone number
  • Lab Information license number, business name, address, phone number
  • Inventory of product to be tested including batch number for all and lot number for edible products
  • Date and Time of departure
  • Transport license number

Without a complete manifest, the sample must be rejected in accordance with current regulations.

2. Valid License Numbers

  • Missouri Facility License Number Missouri Division of Cannabis Regulation (DCR)
  • Facility Agent Identification Number Missouri Division of Cannabis Regulation (DCR)
  • Chain of Custody / Transport Documentation Required when applicable for sample transfers.

If required license numbers or documentation are not provided, the sample may be rejected in accordance with current Missouri Division of Cannabis Regulation (DCR) requirements.

3. Sampling Document

  • Basic Client Information
  • Details of sampling conditions "Intake Form"

Without this document, the sample must be rejected in accordance with current regulations.
